I Don’t Know Which Level of Care I Need

A qualified assessment should guide your level of care. Residential treatment includes living at the facility; PHP and IOP are structured outpatient options with different schedules. Ask which setting can meet your needs safely and what support you would need outside program hours.

Start with your needs, then compare programs

It is understandable to begin with the program that sounds least disruptive. Before choosing, describe what is happening during a typical day and night, what support you have at home, and what previous care has or has not addressed.

Include withdrawal concerns, medical needs, mental health symptoms, substance use, medications, ability to attend consistently, and your living situation. Ask the professional to explain the recommendation in everyday language.

How the daily experience differs

Residential treatment

You live at the treatment setting and participate in a structured program. Ask about staffing, daily activities, clinical services, and how medical or psychiatric needs are handled.

Partial hospitalization: PHP

You attend a structured day program. Ask about actual days and hours, where you will stay, transportation, and support outside treatment hours.

Intensive outpatient: IOP

You attend scheduled outpatient sessions while living outside the program. Ask about attendance requirements, individual appointments, and how the schedule fits your responsibilities.

Program names do not establish that two providers offer identical schedules or services. Confirm the details with each provider.

Questions that help make the recommendation clear

  1. Which needs led you to recommend this level of care?
  2. Can this program safely address my current medical, withdrawal, and mental health needs?
  3. What would I need to manage independently each evening or weekend?
  4. What would tell us that I need more support or can move to less intensive care?
  5. Who reassesses my progress, and how will I participate?
  6. How do benefits, authorization, transportation, and housing affect the plan?

Bring a realistic picture of home

Write down where you would sleep, who is available to help, whether alcohol or drugs are readily accessible, and how you would get to appointments. Describe barriers honestly. A plan that assumes support you do not actually have needs to be reconsidered with the care team.

Also name what is working: a trusted person, a stable routine, a helpful clinician, or a safe place to stay. The assessment should include strengths as well as problems.

What detox and hospital care mean for this decision

If you have immediate medical or psychiatric needs, ask whether you need emergency or hospital care first. If stopping substances may cause withdrawal, seek medical guidance before attempting to stop on your own. Residential treatment should not be assumed to provide hospital-level medical services.

Common questions

Can I choose IOP because I need to work?

Explain your work responsibilities, but ask whether IOP can meet your assessed needs. Discuss other arrangements if the recommended care conflicts with your schedule.

Will I stay at the same level throughout treatment?

Ask how reassessment and transitions work. Your plan may change as needs and circumstances change; get an explanation of the next step before a transition.

When to seek emergency help

Call 911 for immediate danger or a medical emergency, including trouble breathing, inability to wake someone, or seizures. Do not wait for an admissions form or callback. If you may act on thoughts of harming yourself or cannot keep yourself safe, seek emergency care now.

Start with a conversation

Tell Alpine admissions what is happening and what you need help arranging. Ask about assessment, program fit, scheduling, and costs before making a decision.

Admission depends on individual assessment, availability, and applicable benefit or authorization requirements. This guide is educational and does not replace individualized medical or mental health care.
