Screening guide + Alpine preparation checklist • Adults 18+
Look at emotional well-being and learn about the WHO-5 questionnaire.
This is a self-reflection, not a validated screening test. It has no diagnostic score and cannot decide which treatment you need.
WHO-5 uses five statements about well-being during the past two weeks. Higher scores indicate better reported well-being. It is different from a depression diagnosis. The WHO resource includes the official questionnaire and language versions.

Select areas you want to discuss. These are Alpine prompts, not WHO-5 items. You can leave items blank or stop at any time. More checked boxes do not equal a diagnosis or a higher level of care.
No name, email, or phone number is needed. This check-in does not send your choices to admissions or save them in browser storage. General website analytics may still operate. Use a private device when possible. Printing may leave a copy on your device or printer. You choose whether to contact Alpine.
Select the items you want help with. Then choose “Show my next steps.” If you prefer, read the guidance below without making any selections.
You can still read each item and the next steps below. Interactive results need JavaScript.

This result is not a safety clearance or diagnosis. Ask for help whenever you are worried.

Discuss sleep, health, medications, and daily routine.
Choose one small meaningful activity and share ongoing changes with a provider.
Ask for help with both concerns.
Ask a provider which measure and schedule would be useful.
You may ask for support before symptoms become severe.

That does not rule out a concern. You may still ask a qualified professional about anything that worries you.
This check-in does not submit your choices to Alpine. It does not alert a clinician. If you want help, contact a provider yourself.
No. A care provider needs to consider your health, safety, daily life, support, and preferences before recommending treatment.
