Depression Screening: PHQ-9

Check common depression symptoms and get a clear explanation of your result.

This is a screen, not a diagnosis. It cannot tell you which treatment you need. Health, medications, sleep, and substance use may affect your answers.

Start the screen

Before you begin

Answer for yourself. Think about the last two weeks. The questions and answer choices below keep the source wording. You can stop at any time. No clinician is watching your answers or receiving an alert.

No name, email, or phone number is needed. This check-in does not send your choices to admissions or save them in browser storage. General website analytics may still operate. Use a private device when possible. Printing may leave a copy on your device or printer. You choose whether to contact Alpine.

Need urgent help? If someone is in immediate danger, cannot be woken, has trouble breathing, has a seizure, or may have overdosed, call 911. For suicidal thoughts or emotional crisis in the U.S., call 988. Do not wait for an online result or an admissions call. These tools are not monitored.

PHQ-9 questionnaire

Over the last 2 weeks, how often have you been bothered by any of the following problems?

0 of 9 questions answered.

1. Little interest or pleasure in doing things

2. Feeling down, depressed, or hopeless

3. Trouble falling or staying asleep, or sleeping too much

4. Feeling tired or having little energy

5. Poor appetite or overeating

6. Feeling bad about yourself — or that you are a failure or have let yourself or your family down

7. Trouble concentrating on things, such as reading the newspaper or watching television

8. Moving or speaking so slowly that other people could have noticed? Or the opposite — being so fidgety or restless that you have been moving around a lot more than usual

9. Thoughts that you would be better off dead or of hurting yourself in some way

If you checked off any problems, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people?

The daily-life question is not part of your total score.

Interactive scoring needs JavaScript. Use the source questionnaire linked below with a care provider.

Understanding the score

PHQ-9 symptom ranges: 0–4: minimal; 5–9: mild; 10–14: moderate; 15–19: moderately severe; 20–27: severe. These describe reported symptoms. They are not treatment placement rules.

What to tell a care provider

  • When symptoms began and how they affect daily life.
  • Changes in sleep, physical health, medications, alcohol, or drug use.
  • Any safety concerns, even if your total is low.
  • What support you have tried and what you need now.

Questions about this screen

Can I take this for someone else?

No. Use a family checklist for your own observations. This screen asks about your own experience.

Can this tell me whether I need residential care?

No. A clinician needs to review your full situation before recommending a level of care.

What if I skip a question?

No total is shown until all symptom questions are answered. You can stop and discuss concerns with a care provider instead.
